Community Needs Assessment

Please take a few minutes to complete the Community Health Needs Survey.

Every three years, Pike County Memorial Hospital conducts a Community Health Needs Assessment (CHNA). The information obtained from this assessment provides a better understanding of the health needs of our community, gaps in the availability of services, and the impact of behaviors on health outcomes.

Take the Survey...

A key part of a CHNA is gathering input from individuals living across the region and this is where we need your help. Your answers will help Pike County Memorial Hospital to identify the most significant health needs in the communities we serve.

If you have any questions or would like more information about the Community Health Needs Assessment, please contact Lisa Pitzer: or 573-754-5531, ext. 7154.
